Important Notes:
The short term plan has a $250 deductible and then pays 80/20 coinsurance
to $5000.
The permanent plan has a $500 deductible, pays 50% of the next $2500 and
then pays 100%.
The permanent plan includes an $8 prescription drug card (in most states).
Use your permanent home address to obtain the lowest cost (if working in
a high cost area).
There are other variations of coverage (deductibles/coinsurance).
